Twelve children with fever and no localized inflammation were found to have occult pneumococcal bacteremia. In all of these children there was a leukocytosis of more than 20,000 white blood cells per cubic millimeter and a predominance of neutrophils and band forms. Five children presented with seizures. Blood culture is recommended for any child with an unexplained fever and convulsion, particularly if leukocytosis is present.
