We report three cases of persistent cranial nerve palsies secondary to dural puncture despite subsequent epidural blood patching. A review of the literature and the likely mechanism of these palsies indicates that optimal timing of an epidural blood patch to prevent such a complication is no later than the fourth day after onset of the headache.
