This first-of-its-kind systematic review and meta-analysis focused on pediatric populations, providing unique insights into respiratory syncytial virus (RSV) detection methods.The study found that additional testing, beyond the commonly used nasopharyngeal aspirate (NPA), nasopharyngeal swab (NPS), and/or nasal swab (NS) reverse transcriptase polymerase chain reaction (RT-PCR), resulted in only a marginal increase in RSV detection, ranging from 1 to 10% on average, depending on the type of additional specimen.While RT-PCR demonstrated the highest sensitivity, direct fluorescent antibodies exhibited superior performance compared to rapid antigen detection tests and viral culture.In the context of lower respiratory tract infections in children, routine testing using a single NPA or NPS specimen was shown to be effective, suggesting that it is unlikely to miss RSV cases or only detect a minimal proportion.However, the review indicated that incorporating bronchoalveolar lavage or sputum specimens may substantially boost RSV detection.This contrasts with findings in adults, where additional specimen testing significantly increased RSV detection.The observed distinctions between age-groups may be attributed to factors such as the reference specimen type, the collection of lower respiratory specimens, and variations in viral shedding patterns.Strengths of the review include an exhaustive search across diverse literature sources and RSV networks, resulting in a wealth of global data.The inclusion of various clinical settings, presentations, and specimen-testing combinations enhances the generalizability
