Central MessageEn-bloc double lung transplantation (DLTx) with bronchial artery revascularization (BAR) ensures normal airway healing (1).En-bloc DLTx retransplant with BAR was performed to salvage a patient with untreatable airway dehiscence after bilateral sequential LTx for interstitial lung disease (ILD) associated with short telomere syndrome.Donor lungs from a donation after cardiac death (DCD) donor were treated with ex vivo lung perfusion (EVLP).The patient subsequently also recovered from COVID-19 pneumonia.This is the first case of urgent en bloc DLTx with BAR using a DCD donor treated with EVLP.
