The results in terms of penile rigidity and satisfaction are well-established. However, there has been a lack of proper evaluation of patients' quality of life and male genital image after PPI. METHODS: Single cohort descriptive cross-sectional study. After approval by the Clinical Research Ethics Committee, patients with PPI for more than 6 months and who had been sexually active in the last 3 months, were considered. Patients were contacted by telephone and invited to participate. Subsequent personal interview and clinical examination were performed. Data about surgical procedure, type of penile implant, short, medium and long term complications, and objective evaluation of the genital appearance (including penile length and girth), were collected Self-administered questionnaires: Quality of Life and Sexuality with Penile Prosthesis (QoLSPP), Index of Male Genital Image (lMGI) and International Index of Erectile Function (IIEF-15). Data collection: July to November 2021 in 3 Andrology Reference Hospitals. Descriptive statistical analysis using STATA 13.1 for Mac. RESULTS: Cohort: 101 patients. Age at PPI: 58.7 (IQR 54.6-63.1) years old. Time since implantation: 32.8 (IQR 22.3-47.5) months. Penile implant type: 6 malleable, 94 3-piece and 1 2-piece. Aetiology: Post-prostatectomy 27.7%, Diabetes Mellitus: 25,7%, Peyronie disease 11,9%. Pre-surgical IIEF-5: 7 (IQR 5-12). Surgical time: 100 (IQR 85 - 120) minutes. Intra-operative complications: 7.9%. Post-operative: 2 complications grade III (Clavien-Dindo Classification). 4 revision surgeries. Penile length: 11.6 (SD 2.0) cm. Pre- pubic fat: 1 (IQR 0.5-2.5) cm. Penile girth: 10.5 (IQR 10 - 11.5) cm. Tubes/connections visualization: 20.8%. SST deformity: 5.9%. Circumcision: 50.5%.Total QoLSPP score: 69 (IQR 55-74), functional domain: 22 (IQR 19-24), personal: 17 (IQR 12-19), relational: 16 (IQR 12-18) and social: 13 (IQR 11-15). Total IIEF-15 score: 67 (IQR 56-73), erectile function domain: 29 (IQR 25-30), orgasmic function: 9 (IQR 6- 10), sexual desire: 7.3 (SD 2.1), relationship satisfaction: 11 (IQR 9-13) and overall satisfaction: 8 (IQR 7-10). IMGI score: 70.1 (SD 14.6). CONCLUSIONS: Our study confirms that PPI surgery has a highly positive impact on sexual function. Moreover, device-related quality of life has been shown to be satisfactory, and patients report a positive male genital self-image, despite post-surgical body changes.
