The purpose of this study is to outline national trends in the use of Minimally Invasive Surgical Therapy (MIST) for BPH/LUTS and associated retreatment rates. METHODS: Using national Medicare fee-for-service data from 2012-2021, we identified men aged 65 years and older who received a claim-based diagnosis of BPH and associated lower urinary tract symptoms in 2015, without having a claim-based diagnosis in the 3 years prior (i.e., incident BPH/LUTS). Men in this incident cohort were tracked for 4 years to assess the proportion undergoing transurethral surgery or MIST and the percentage experiencing retreatment within 2 years following their initial BPH/LUTS surgery. Men with retreatment within 30 days were excluded from the analysis. RESULTS: In 2015, there were 317,155 newly recorded cases of BPH/LUTS among men. Out of these individuals, 19,410 (6%) underwent surgery for BPH/LUTS within four years of their diagnosis. Among this group, 14,768 (76%) underwent transurethral surgery, while 4,642 (24%) had MIST. Overall, 1,154 (6%) men required retreatment within two years following their initial surgery, including 596 (4%) of the men who had transurethral surgery and 558 (12%) of those who had MIST. The retreatment rates for transurethral surgery ranged from 3% to 4%, whereas for MIST, they varied from 9% to 16% (Figure 1). CONCLUSIONS: In this cohort of Medicare patients, those who underwent MIST for BPH/LUTS exhibited higher rates of retreatment compared to transurethral surgeries. Enhancing the durability of MIST should be an important clinical and research priority.
