Our previous study found prostatic inflammation and systemic inflammation were correlated. Diet may alter the systemic inflammation, thus contributing to LUTS/BPH. Another previous study demonstrated a positive correlation between LUTS and Dietary Inflammatory Index (DII) among men in the US. This study aimed to investigate epidemiologic associations between DII and LUTS/BPH among Asian population and underlying biologic mechanisms. METHODS: Dietary surveys were conducted among LUTS/BPH patients by food frequency questionnaire. Serum levels of DII-related systemic pro-inflammatory biomarkers were measured. To evaluate the association between DII and prostate volume and International Prostate Symptom Scores (IPSS), we used univariate and multivariate regression analyses. To explore the mediation effect of pro-inflammatory biomarkers on the association between DII and LUTS/BPH, we conducted the causal mediation analysis using quasi-Bayesian approximation (Figure 1). RESULTS: A total of 117 BPH patients were included in the analysis. Compared with patients in the lowest DII quintile, those in the highest DII quintile had larger prostates and higher IPSS. A 1-unit increase of DII was associated with a 5.14 cm3 increase in prostate volume and a 1.05-point increase in IPSS in the multivariate analysis. Besides, serum TNF-α mediated 39% of the correlation between DII and prostate volume and 34% of the correlation between DII and IPSS (Table 1). CONCLUSIONS: A higher dietary inflammatory potential was associated with a larger prostate and more severe LUTS. Systemic TNF-α is a potential mechanism linking dietary inflammation to LUTS/BPH.
