In this study, we aimed to characterize the genomic and transcriptomic landscape of Chinese UTUC patients and explore the prognostic markers of adjuvant chemotherapy. METHODS: Overall, 44 Chinese patients with UTUC receiving adjuvant chemotherapy after radical surgery were enrolled. Genomic DNA and total RNA and were extracted from sample cells. Whole exome sequencing and RNA sequencing were then performed. Mutational subtype, Expression clustering and Tumor Micro-Environment component were further analyzed. RESULTS: In total, 22 patients were treated with Gemcitabine+Cisplatin and the rest received Gemcitabine+Carboplatin. 39 of 44 (88.6%) patients developed relapse or metastasis during follow-up. Genomic landscape were illuminated and these patients could be classified into 5 mutational subtypes: hypermutated, TP53-mutated, RAS-mutated, FGFR3-mutated and triple-negative. Patients with TP53-mutated/triple-negative subtype tend to have a worse PFS (p=0.024). Furthermore, transcriptomic landscape of these patients were demonstrated and they were classified into four expression type: C1, C2, C3 and C4. In addition, TME estimation was performed based on RNA-expression data by MCPCounter Method and QuanTISeq Method. It showed that contexture fraction of neutrophils was a prognostic factor for PFS. Patients with less neutrophil infiltration have a wors PFS after adjuvant chemotherapy (p=0.005). This was further confirmed by immunohistochemical results. CONCLUSIONS: This study demonstrated the genomic and transcriptomic landscape of Chinese UTUC patients for the first time. These data emphasized the importance of genetic testing in UTUC patients, which will help guide adjuvant chemotherapy and improve clinical decision making.
