Some reports highlighted that almost half of patients with nmCRPC based on conventional imaging might shift to CRPC when reassessed with PSMA PET scan. Nowadays, patients with oligometastatic (om) castration resistant prostate cancer (CRPC) usually receive treatments targeting androgen receptors, similarly to nmCRPC patients. Based on these data, in this study we compared oncologic outcomes of patients with nmCRPC and oligometastatic population from a longitudinal real-life multicenter series. METHODS: We retrospectively analyzed data extracted from chemo na'ive CRPC and from nmCRPC dataset. The former was queried for low volume disease (defined as non metastatic disease or less than 6 bone lesions or bulky node25 cm), the latter for all nmCRPC patients. We compared the main clinical features of nmCRPC versus omCRPC patients. Mann-Whitney test and Chi square test were used to compare continuous and categorial variables, respectively. The Kaplan-Meier method was used to compare progression-free (PFS), cancer-specific (CSS) and overall survival (0s) probabilities between groups. Survival probabilities were computed at 12.24.48 and 60 months. RESULTS: Overall, 48 (27%) nmCRPC and 130 omCRPC patients (73%) were included in the analysis. Overall, 5-yr PFS. CSS and OS probabilities were 61.9%. 72.3% and 69.1%. respectively. All these cases were treated with antiandrogens receptor targeted agents as first treatment line. The oligometastatic cohort was younger (p=0.034), displayed a higher PSA level (p=0.023) and a higher rate of nodal metastatis (p=0.011) than the non-metastatic cohort.The nmCRPC and the omCRPC cohorts displayed comparable PFS probabilities after the first treatment line (log rank p=0.317, Figure 1).At kaplan meyer analysis, CSS and OS probabilities were comparable for nmCRPC and omCRPC group (log rank p=0.43 and p=0.93, respectively). CONCLUSIONS: In this multicentre real-life sieries, nm- and om-CRPC cohorts displayed comparable progression free survival probabilities at first line treatment. These data shed lights on actual limits of imaging modalities and raise the question about the existence itself of nmCRPC patients.
