Peripheral artery disease is a prevalent but underdiagnosed manifestation of atherosclerosis. There is limited data available on the impact of anemia on the peripheral artery disease (PAD) outcomes. We sought to examine the national inpatient sample database to describe in-hospital outcomes among these patients. Data were extracted from the National Inpatient Sample (NIS) Database for the years 2019 and 2020. The NIS was searched for hospitalizations of adult patients with PAD with and without a concomitant diagnosis of anemia using international classification of diseases 10th revision codes. Multivariate logistic was used to adjust for confounders. The primary outcome was inpatient mortality. Secondary outcomes were hospital length of stay (LOS), and total hospital charges (TOTHCG). SPSS software was used for statistical analysis. This study included 479, 038 patients with PAD, of which 244, 493 (51. 0 %) patients had anemia. PAD disease patients with anemia had higher prevalence of smoking (27. 2% Vs. 25. 3%, p<0. 001), obesity (15. 3% Vs. 10. 4%, p<0. 001), and chronic kidney disease (3. 1% Vs. 1. 9%, p<0. 001). In-hospital mortality was higher among the anemic cohort (4. 7% Vs. 4. 0%, p< 0. 001). Multivariate regression showed that patients with both anemia and PAD had higher inpatient mortality (OR 2. 276, CI 2. 240-2. 313, p<0. 001) in addition to paying significantly more total hospital charges (80, 478 Vs. 43, 882, p<0. 001). The mean length of stay (LOS) was more for patients with PAD and anemia (6. 8 days Vs. 5. 4 days, p<0. 001). On secondary analysis it has shown that patients with PAD and anemia had higher odds of having myocardial infarction (OR 2. 721, CI 2. 636-2. 809, p<0. 001), acute kidney injury (3. 159, CI 3. 132-3. 186, p<0. 001), congestive heart failure exacerbation (OR 2. 898, CI 2. 809- 2. 914, p<0. 001), acute hepatitis (OR 2. 040, CI 1. 986- 2. 095, p<0. 001), and arrhythmias (OR 2. 277, CI 2. 259-2. 296, p<0. 001) compared to PAD alone. In this nationally representative population-based retrospective cohort study, anemia was associated with higher mortality and worse outcomes among patients with PAD.
