Crohn’s Disease (CD) is a chronic inflammatory condition of the gastrointestinal tract with variable presentation, complicating early diagnosis, especially in children and adolescents. This report describes a 14-year-old male patient with iron-deficiency anemia, abdominal pain, and intermittent fever, diagnosed with a severe form of CD after multiple hospitalizations without a conclusive diagnosis. Capsule endoscopy revealed extensive ulcerations in the duodenum and jejunum, confirming the severity of the disease. Treatment with Adalimumab resulted in a positive clinical and laboratory response, with the patient becoming asymptomatic and gaining 7 kg in six months. This case highlights the importance of early diagnosis and the efficacy of biological treatment in inducing remission and promoting healthy growth in pediatric patients.
