Background: Amid health system reform calls, five provincial organizations launched initiatives to transform primary care and build practices' capacity and capability to respond to an increasingly complex healthcare environment and move towards an integrated system.Audience: This workshop is for leaders, managers, advisors, champions, and policy-makers of provincial groups and primary care organizations. Approach: This workshop demonstrates how to design and implement practice support initiatives building towards an integrated primary healthcare system. Five case studies will provide a roadmap for successful primary healthcare system capacity for integration that achieves the Quintuple Aim.Case study is Newfoundland and Labrador Family Practice Renewal Program MyQ Quality in Family Practice aimed at transforming family practice to achieve better health outcomes. The program aligned provincial partners to offer capacity-building diverse supports, including three integrated care demonstrations. Case study 2 is Nova Scotia Health Practice Support Program (PSP), aligning the Primary Health Care (PHC) and Chronic Disease Management Network to co-lead with other system partners to build a high-performing primary healthcare system towards a Health Home model within the Health Neighborhood. Case Study 3 is the New Brunswick Medical Society PSP, supporting the transformation of NB PHC system towards the Patient Medical Home. NBMS is using evidence-based approaches, leadership alignment, capacity building for practice facilitators and PHC teams, and leveraging existing system enablers such as EMRs and panel reports. Case study 4 is Alberta Palliser Primary Care Network Health Home Expedition for family physicians and interdisciplinary teams. The PCN supports member practices to implement a Health Home model of care using aligned leadership, team resources, and practice facilitation.Case study 5 is eHealth Centre for Excellence, a not-for-profit digital health organization that improves the quality of patient care through the effective and innovative use of enabling technologies, working with Canadian healthcare professionals to improve the use of digital health. Three pivotal elements for integrated care were leveraged in case studies: . Visible Leadership: Aligning and coordinating primary healthcare and provincial organizational leaders to establish a shared vision, transformation roadmap, and comprehensive indicators/measures for healthcare transformation.2. Capacity Building at provincial and practice levels: Focusing on building the health transformation workforce; designing and implementing practice support for primary healthcare teams through education, practice facilitation, tailored tools, and resources.3. System enablers: Providing essential infrastructure, policy supports, system-level data, and new funding/compensation models for interdisciplinary teams. This 90-minute interactive workshop, including participant workbooks, utilizes five case studies and guides participants through group discussions and activities leading to applied contextual learning.
