A BSTRACT An uncommon malignant salivary gland tumor is called epithelial–myoepithelial carcinoma. Less than 1% of all salivary gland neoplasms are caused by it. Donath et al . initially described these uncommon salivary gland tumors in 1972. This tumor, which is more common in women, develops in the sixth decade and beyond. Parotid gland is the most commonly involved followed by submandibular gland and palate. This is a rare instance of parotid gland epithelial–myoepithelial carcinoma. A 43-year-old female presented with a complaint of growth over the left side of the face for 8 years, aggravated 1 year ago, and she was evaluated with relevant investigations and underwent left parotidectomy + left modified radical neck dissection III + ALT flap. According to histopathology report, patient was planned for adjuvant radiation therapy with 66 Gy in 33# over 6.5 weeks at 2 Gy/# with intensity modulated radiation therapy technique. The patient is on follow-up since 1 year with no complaints.
