The increasing use of intrauterine contraceptive devices (IUCD) over recent decades has led to more detection of rare complications, including bizarre migrations of IUCD, some requiring immediate surgical help. A young G3P2L2 female with unwanted pregnancy due to contraception failure, with 9 weeks of amenorrhoea. Ultrasound showed an 8-week singleton intrauterine pregnancy with an exteriorised IUCD abutting the anterior uterine wall with sigmoid adhesions and omental wrapping.The patient had a medical termination and exploratory laparotomy. During surgery, the IUCD was observed to be abutting the sigmoid colon with dense adhesions. The IUCD was removed, the defect repaired, and a modified Pomeroy tubectomy was performed. IUCDS are popular due to ease of use and reversibility, but can cause serious complications requiring major surgery.
