Motivation: Preoperative prediction of microsatellite instability (MSI) can provide a basis for personalized and precise treatment of endometrial cancer (EC). Goal(s): This study aimed to determine the feasibility of amide proton transfer-weighted imaging (APTw) and T2 mapping parameters in assessing MSI in EC. Approach: A total of 58 patients underwent preoperative MRI. Results: The APTw and T2 values were significantly different in MSI and microsatellite stable (MSS) groups. The AUC value of the combined APTw and T2 mapping model was significantly higher than the individual parameters. Therefore, the combined model might be considered effective in assessing the MSI of EC preoperatively. Impact: Our results demonstrate the promising potential of this combined model for the preoperative assessment of MSI in EA, which assists in formulating clinical therapeutic strategies and assessing prognosis.
