Abstract Labial adhesions and vaginal stenosis are rare but distressing sequelae in postmenopausal women, typically related to estrogen deficiency. These may result in significant urogenital dysfunction, including urinary obstruction and sexual difficulties. We present a case of severe postmenopausal labial adhesions with concomitant urethral obstruction and vaginal stenosis successfully managed by surgical intervention, supported by topical estrogen therapy and postoperative counseling. The case underscores the clinical vigilance required for such presentations and the effectiveness of multidisciplinary management in restoring functional anatomy and quality of life.
