The aim of this study was to evaluate the therapeutic efficacy and safety of immunotherapy beyond progression in advanced gastric cancer (GC) after the failure of first-line immunotherapy in combination with chemotherapy, with a view to finding a more effective alternative therapeutic regimen for this group of patients. We conducted a retrospective study to evaluate the efficacy and safety of immunotherapy beyond progression in advanced gastric cancer. Response evaluation criteria in solid tumors (RECIST 1.1) were utilized to assess the local response of gastric cancer. Progression-free survival (PFS) and overall survival (OS) were the primary outcomes, while objective response rate (ORR), disease control rate (DCR), and adverse events (AEs) were the secondary ones. A total of 41 patients were enrolled in the study. No patients achieved complete response, while 10 patients achieved partial response. Furthermore, 18 patients achieved stable disease and 13 patients achieved progressive disease. The overall ORR and DCR were 24.4 % (10/41) and 68.3 % (28/41), respectively. The median PFS for immunotherapy beyond progression in advanced GC was 4.1 months. The median OS was 7.0 months. Immunotherapy beyond progression shows potential clinical efficacy in advanced GC and provide a new treatment option for patients with advanced GC. However, the relevant results still need to be treated with caution.
