Abstract Extracranial carotid artery pseudoaneurysms are rare. Here, we describe a patient with thrombosed carotid artery pseudoaneurysm managed successfully with open repair. This case highlights the importance of how an intraoperative finding can alter the surgical plan from a carotid ligation with the risk of stroke, to open repair and bypass, with no resultant neurological deficit.
