Background: Thoracic meningocele is a rare complication of neurofibromatosis type 1 (NF1), and only rarely do symptomatic lesions require surgery. Here, a 55-year-old female with NF1 and an 80 mm T4 thoracic meningocele was successfully managed with the placement of a thoracic cystoperitoneal (CP) shunt. Case Description: A 55-year-old female with NF1 presented with progressive dyspnea and cough. The magnetic resonance (MR)-documented left lung compression due to a T4 80 mm thoracic meningocele. Due to the anticipated high morbidity of an open surgery, the patient underwent CP shunt placement. The cyst puncture route was determined based upon both computed tomography (CT) and MR studies and supplemented with intraoperative ultrasound. To minimize intrathoracic pressure fluctuations, one lung was collapsed while the other was routinely ventilated. Once the shunt catheter was safely and accurately placed, the patient’s respiratory symptoms improved, and she remained symptom-free 6 months later. Conclusion: Utilizing preoperative CT/MR and intraoperative ultrasound guidance, a 55-year-old female underwent safe/effective CP shunting of a T4 80 mm thoracic meningocele attributed to underlying NF1.
