Abstract Background Endoscopic balloon dilation (EBD) is a minimally invasive treatment for Crohn’s disease (CD)-associated bowel strictures. This large multicenter cohort study aims to evaluate the effectiveness and safety of EBD and identify predictors of reintervention. Methods We analyzed 156 CD patients with symptomatic strictures undergoing EBD across two hospitals. Primary outcomes included technical success, clinical response at 8 weeks, reintervention-free survival, and complications. Results The technical success rate was 92.1% (139/151) per patient and 91.9% (182/198) per procedure. Early clinical response occurred in 86.2% of patients. The cumulative reintervention-free rates were 84.5% (95% Confidence Interval CI: 78.6–90.9%) at 1 year, 78.8% (95% CI: 71.8–86.4%) at 2 years, 74.2% (95% CI: 66.5–82.7%) at 3 years, 59.4% (95% CI: 49.5–71.2%) at 5 years, and 45.6% (95% CI: 33.5–62.1%) at 10 years. Penetrating (B3) disease (Hazard Ratio HR = 1.82, P = 0.047) and absence of clinical response within 8 weeks (HR = 0.40, P = 0.0088) independently predicted reintervention. Major complications were rare (4.5% per-patient). Conclusion This study demonstrates high technical success, favorable short- and long-term effectiveness, with nearly half of patients avoiding reintervention over a 10-year period. Early clinical non-response and penetrating disease behavior help identify high-risk patients, aiding in personalized treatment sequencing for stricturing CD.
