Background: As a comprehensive stroke certified center, the success of patient care relies on a multidisciplinary team approach. The acute phase of stroke care involves the stroke team and the Neuro critical care (NCC) team. Post discharge from the NCC unit, the team may not receive updates on the patient's outcome. To bridge this gap, we implemented a detailed case report presentation during the monthly NCC meetings. We sought to inform the ICU team by providing closure and recognizing their efforts. Additionally, this highlights our multidisciplinary approach by fostering a shared understanding of patient outcomes after leaving the ICU. Smart Aim: Using a Likert Scale, a 3 Question survey was conducted by the attendees. The goal was that the NCC team would find monthly Stroke Patient Presentations 80% beneficial in 3 areas. Meaningful, beneficial&impact in care. Implementation: The stroke RN began comprehensive case presentations at the NCC monthly team meeting. Including, all MDs, NPs and NCC Nursing Management. Presentations included brain imaging, procedures the patient received, their course of stay in NCC and patient outcome. After 4 months of presentations, the NCC team was asked 3 Likert questions to gauge the benefit. Mthods/Implementations: The first question asked was “How helpful do you find the patient case presentations at the NCC meeting?” 92% strongly agreed, while 8% Agreed. When asked the second question “Do you find the feedback and closure on your ICU patients meaningful?” 100% strongly agreed. The third Question “Do you find the post discharge presentation to have any impact on the ICU patient population?” 92% strongly agreed and 8% agreed. The Likert Scale survey results show that the presentations to the Neuro Critical Care team exceeded our goal of 80% beneficial. Conclusion: In Conclusion, the monthly patient presentations have proven to be a helpful, meaningful and impact practice for the NCC team. By providing updates on patient outcomes after leaving the ICU, these sessions allow MDs/NPs, and nursing management to see the long-term impact of the critical care they provided. Allowing for a sense of closure, satisfaction and provides feedback that can contribute to improved patient care and outcomes in the future. Sharing outcomes and highlighting successes of the multidisciplinary stroke team reinforces the importance of the ICU team's work, promotes continuous learning, and further strengthens the multidisciplinary teams’ efforts.
