Abstract Introduction Problematic pornography use (PPU) has been increasing, and people suffer from its consequences, while debates remain inconclusive about whether PPU is an obsessive-compulsive behavior problem, an impulse control problem, or a behavioral addiction. But how can we treat those suffering from the harms of PPU? What is known about the role of cognitive-behavioral therapy in treating pornography abuse? Objectives To analyze Cognitive-Behavioral Therapy (CBT) protocols in the treatment of PPU, a scoping review was conducted, based on the PRISMA ScR protocol, in the Scopus, Web of Science, PsycInfo, and PubMed databases, of documents published between 2019 and 2024. Results Considering the search terms used and inclusion and exclusion criteria, 435 articles were retrieved from the PubMed (6), Scopus (258), and PsycInfo (171) databases. The articles selected for the study were analyzed into categories. Conclusion CBT-based interventions for PPU showed reductions in frequency of use, cravings, and emotional distress, even without achieving abstinence. Mindfulness was widely used, but associated only with emotional regulation. The lack of consensus on definition, diagnostic criteria, and protocol focus hinders clinical efficacy and scientific communication. Questions persist about the feasibility of functional and controlled pornography use and the role of abstinence in treatment. Financing No conflict.
