Rwanda's district hospitals play a critical role in healthcare delivery, but their efficiency is poorly understood. A mixed-method approach including quantitative data collection and qualitative interviews was employed to assess system performance over a two-year period. District hospitals reported an average 20% reduction in patient wait times, with significant variance among regions. The randomized field trial methodology provided insights into cost-effectiveness but also highlighted disparities across districts. Further longitudinal studies should be conducted to understand long-term impacts and to tailor interventions for specific areas. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
