District hospitals in Uganda face challenges related to resource allocation and service delivery efficiency. A randomized field trial was conducted among five randomly selected district hospitals, employing statistical models for data analysis. The intervention led to an average yield improvement of 20% in resource utilization (95% CI: 15-25%). The randomized trial demonstrated significant potential for improving hospital systems efficiency with further research and implementation. Future studies should focus on scalability and cost-effectiveness, while ensuring sustainability of improvements. district hospitals, yield improvement, randomized field trial, resource allocation, Uganda Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
