District hospitals in Ghana play a crucial role in healthcare delivery, but their efficiency varies widely. A systematic literature review was conducted using databases such as PubMed and Google Scholar. Studies were screened based on predefined criteria and assessed for quality using a validated tool. The analysis revealed that most studies employed mixed-methods designs, with qualitative data often being used to supplement quantitative findings. Current evaluations of district hospital systems in Ghana lack robust statistical models, which limits the generalizability of their cost-effectiveness assessments. Future research should prioritise the implementation of standardised econometric models to enhance the reliability and validity of cost-effectiveness analyses. District Hospitals, Cost-Effectiveness, Methodological Evaluation, Ghana Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
