This paper proposes that depression is not a serotonin deficiency but a gate calibration problem with four independent biological pathways each producing identical limbic signal dominance at the thalamic reticular nucleus. Inflammatory, cortisol, dopamine, and transcriptional subtypes each require a different intervention — explaining why a single pharmacological approach produces remission in only one third of patients and fails the rest. The framework generates a diagnostic approach using existing clinical tools to identify signal source before prescribing treatment, converting treatment resistant depression from a mysterious clinical phenomenon into a solvable engineering problem.
