Etiology to Intervention: The Clinical Significance of Vikara Samutthana in Ayurvedic Diagnostics Within the Trividha Bodhya Sangraha Framework

Introduction: Precise clinical diagnosis in Ayurveda relies on a structured understanding of pathogenesis. Acharya Charaka’s Trividha Bodhya Sangraha provides a robust diagnostic triad: Vikara Prakruti (pathological nature), Vikara Adhisthana (anatomical site) and Vikara Samutthana (etiological origin). While this framework is vital for identifying diseases, it is indispensable for diagnosing Anukta Vyadhi (unclassified disorders). This study focuses on Vikara Samutthana as the primary driver for therapeutic strategy. Methods: A comprehensive literary analysis was conducted using the Charaka Samhita, Ashtanga Hridaya, Madhava Nidana etc and their respective commentaries. The study synthesized classical concepts of Nidana Panchaka and Samprapti Bheda to evaluate their clinical integration with Vikara Samutthana. Results: The investigation reveals that Vikara Samutthana serves as a chronological roadmap of disease, spanning from initial Nidana Sevana (etiology) to Dosha-Dushya Sammurchhana (pathological synergy). By applying Samprapti Bheda, a clinician can quantify disease severity and predict prognosis (Sadhya-Asadhyata). Identifying the Samutthatva- whether originating in the Amashaya (stomach) or Pakvashaya and Samutthana are the decisive factor in selecting the appropriate Shodhana (purification) or Shamana (palliation) protocol. Conclusion: The Trividha Bodhya Sangraha framework, centered on Vikara Samutthana, transforms theoretical pathology into actionable clinical practice. Mastering the origin and evolution of a disease enables Samprapti Vighatana (disruption of pathogenesis), ensuring a systematic transition from state of illness to Prakruti Sthapana.
