This paper proposes two axioms — Ω₀ (consciousness is a phase transition of information) and Ω₁ (information connects non-locally) — to simultaneously address the 127-year anesthesia mystery and the question of information at death. From Ω₀ together with two independently grounded physical principles (Landauer's principle and Unitarity), the framework derives Theorem T1 (Erasure = Transfer) and a closed-loop interpretation m ⇄ E ⇄ I ⇄ C with no fitted free parameters in the primary derivation. Key quantitative results: three convergences at 1. 00 from thermodynamics, quantum mechanics, and neuroscience (with structural anti-correlation between two of them derived in Appendix D) ; species gamma frequency predictions 12. 8× more accurate than classical alternatives under identical 2-anchor calibration; 8/8 directional retrodictions matching existing observations; 16/16 internal consistency checks satisfied. The framework is explicitly falsifiable via two numerical thresholds (mouse gamma > 200 Hz or Fₜotal < 10⁶). Cross-domain corroboration includes 1/1 prospective verification of the A2 Cancellation Theorem in optical phase singularities (Bucher et al. , Nature 2026). Full simulation code and verification scripts are provided on Zenodo for complete reproducibility.
