A 90-year-old male with epithelioid angiosarcoma initially presenting as a difficult-to-diagnose temporal mass developed lung metastasis 3 months after excision and radiation therapy.
Case Report
90-year-old male with a purplish-red, flat elevated mass on the right temporal region, ultimately diagnosed with Epithelioid angiosarcoma (EAS)
Skin tumor excision, full-thickness skin grafting, and radiation therapy
Epithelioid angiosarcoma can present with diverse symptoms making clinical diagnosis difficult, sometimes requiring complete excision for definitive histopathological and immunohistochemical diagnosis.
90 歳，男性。初診の 3 カ月前より右側頭部の腫脹と排膿に気が付いた。初診時，右側頭部に 30×23 mm の紫紅色，扁平に隆起した腫瘤を認めた。生検を行い，真皮全層に赤血球の漏出と炎症細胞の浸潤を認めたが，腫瘍性病変は確認できなかった。創部は一旦消失したが，初診後 1 年 5 カ月頃より同部位が隆起するようになった。悪性腫瘍の可能性も考えられたため再生検を行ったが診断がつかず，治療および診断目的に皮膚腫瘍摘出術を行った。病理組織学的所見では，皮下組織を主座に類上皮様の腫瘍細胞の充実性シート状増殖を認め，部分的に血管様構築がみられた。免疫組織化学的所見では腫瘍細胞は CD31，D2-40，ERG が陽性を示し，Epithelioid angiosarcoma（EAS）と診断した。深部断端陽性のため二期的に皮膚悪性腫瘍切除術および全層植皮術を施行した。その後，放射線療法を施行したが，放射線療法終了後より 3 カ月後の CT 検査にて肺転移が認められた。患者は抗癌剤治療を希望せず緩和治療を行う方針となった。EAS は多彩な症状を呈するため臨床所見からの診断を困難とする報告が多く，文献をもとに EAS の臨床類型について考察した。
