This work proposes a minimal ontological clarification of physical reality, based on the observation that established physical theories impose structural limits on informational accessibility. It formalizes the distinction between global physical states and locally accessible descriptions, without introducing modifications to the formalism of quantum mechanics or general relativity. The framework is derived from structural features already present in contemporary physics, including quantum decoherence, entanglement, and gravitational horizons, which collectively indicate a dissociation between global ontological states and effective local descriptions. Rather than introducing new dynamics, the approach makes explicit an implicit structural constraint: physical reality may include degrees of freedom that are causally active yet inaccessible within a given observational regime. The framework is empirically constrained through a falsifiable criterion based on the reconstructibility of quantum coherence, allowing it to be distinguished from purely interpretative reformulations. This work aims to provide a minimal and coherent conceptual structure unifying phenomena such as superposition, measurement, decoherence, entanglement, and gravitational horizons within a single accessibility-based perspective.
