The biopharmaceutical sector represents one of the most dynamic and rapidly evolving components of the Indian healthcare and biotechnology ecosystem. In the present Indian context, characterized by demographic transition, epidemiological shifts toward chronic diseases, and increasing technological integration, biopharmaceuticals have emerged as a critical driver of therapeutic innovation, economic growth, and global healthcare competitiveness. This review article systematically examines the structural evolution, market dynamics, regulatory frameworks, technological advancements, and translational challenges associated with the biopharmaceutical industry in India. India’s bioeconomy has reached approximately USD 195.3 billion in 2025, contributing nearly 5% to the national GDP, with significant expansion driven by biosimilars, vaccine manufacturing, and biotechnology startups. Concurrently, the biopharmaceutical segment alone is projected to grow at a compound annual growth rate of approximately 8.3% through 2033, reflecting increasing domestic demand and export potential. The proliferation of over 11,000 biotech startups, alongside robust government initiatives such as the BioE3 policy and BIRAC support, has strengthened India’s position as a global hub for affordable biologics. This review critically evaluates key segments including monoclonal antibodies, recombinant therapeutics, vaccines, and biosimilars, while also analyzing translational research, regulatory challenges, pricing dynamics, and ethical considerations. The article further explores emerging trends such as precision medicine, artificial intelligence in drug discovery, and contract research and manufacturing organizations (CRDMOs). The findings underscore that while India possesses significant advantages in cost-efficient manufacturing, skilled workforce, and global supply chain integration, persistent challenges related to regulatory harmonization, R&D investment, and intellectual property frameworks remain. A strategic alignment of policy, innovation, and infrastructure is essential for India to transition from a volume-driven to a value-driven biopharmaceutical leader in the global arena.
