Introduction Light adjustable intraocular lenses can provide excellent refractive outcomes in patients who need cataract surgery and corneal endothelial surgery for conditions such as Fuchs dystrophy. Descemet stripping only is an effective endothelial surgery that can be combined with cataract surgery. Case Report A 58-year-old male patient with a history of myopic laser-assisted in situ keratomileusis, for distance vision in the right eye 15 years ago, presented desiring reading vision in his left eye. He was noted to have Fuchs dystrophy and a mild cataract in both eyes. To achieve the patient’s goals of functional near vision, the patient was referred for corneal and refractive lensectomy in his left eye. He was successfully treated with a combined Descemet stripping only procedure and phacoemulsification with light adjustable lens implantation. After the cornea had cleared, he was able to have his left eye adjusted to his preferred near prescription. His final uncorrected distance visual acuity was 20/20 with both eyes. His final uncorrected near visual acuity was 20/25 with both eyes. Conclusion This case shows how light adjustable lens technology can be used for challenging refractive cases. Intraocular lens power selection is more challenging in combined cornea and lens surgeries . This patient achieved excellent blended vision with a single surgery that addressed both his corneal pathology and desire to be glasses independent.
