Abstract Background and aims Guidance from JCVI and the Green Book (Autumn/Winter Campaign) recommend that patients with chronic neurological disease including stroke and TIAs (among others) receive both Influenza and COVID vaccines as these currently provide short term protection. Patients under the ASU are often inpatients for weeks to months, hence access to vaccinations with their GP is impacted. Offering the vaccines to eligible inpatients help contribute to the aim of the programmes to reduce COVID disease severity and directly protect those in groups at higher risk of influenza-associated morbidity and mortality. Methods A direct observational study of hospital records on EPIC was conducted on the 38 patients admitted in NHNN under ASU from January to March 2025 to determine if Influenza and COVID vaccines were offered (confirmed through patient notes) and, administered (confirmed through prescription). All 38 patients were diagnosed with a stroke, indicating eligibility of the entire cohort for the vaccines. Results Only 18% (7/38) were offered Influenza or COVID-19 vaccination whilst inpatient. Of the 7 vaccinations offered, only 5 (13%) had been administered whilst admitted to NHNN, 1 had refused (reason undocumented), and 1 had the vaccine administration cancelled (reason undocumented) Conclusions There remains significant room for improvement in offering and administering either vaccinations, as well as its relevant documentation protocols. An educational intervention for residents on vaccine-offering protocol and counseling was conducted to ultimately improve inpatient vaccination within stroke units. Another cycle for patients admitted between December 2025 and February 2026 is currently being conducted to measure the effectiveness of this.
