Abstract Introduction Comfort and temperature regulation play essential roles in sleep quality. Mattresses that provide enhanced cooling, support, and pressure relief may help improve sleep continuity and restfulness. This study compared sleep on a premium cooling mattress to participants’ baseline sleep on their original mattresses. Methods Healthy adults (N=19; 94% female; M age=40) who reported sleeping hot or experiencing nighttime temperature discomfort participated in a 10-week in-home pre–post study. Participants slept on their original mattresses for four weeks, followed by six weeks on a Sealy Posturepedic Elite Mattress, including a two-week adjustment period. Objective sleep was measured nightly using a PSG-validated, noncontact biomotion device, SleepScore Max. Daily and end-of-period surveys assessed perceived comfort, pressure relief, cooling, support, tossing and turning, pain, and overall sleep quality. Analyses included multilevel regression and paired t-tests (α=.05). Results Across 699 tracked nights, participants spent more time in bed (+3%) and showed significant objective improvements in total sleep duration (+5%), SleepScore (age- and gender-normalized overall sleep quality; +4%), MindScore (age- and gender-normalized REM sleep; +6%) and REM sleep (+14%). They also exhibited higher sleep efficiency (+2%), greater sleep maintenance (+2%), fewer awakenings (–8%), and less time awake after sleep onset (–16%). All ps.05. Self-reported outcomes improved substantially, including comfort (+124%), support (+177%), pressure relief (+283%), cooling (+175%), and sleep quality (+226%), along with reduced tossing and turning (+199%) and aches and pains (–52%). Participants also reported higher sleep satisfaction (+230%) and feeling well-rested in the morning (+200%) All ps.05). Conclusion Sleeping on a premium cooling mattress was associated with significant objective and self-reported improvements in sleep quality, comfort, and perceived cooling compared to participants’ original mattresses.
