Nasopharyngeal carcinoma (NPC) is a rare, yet aggressive malignancy characterized by various clinical presentations. Metastasis to the orbital and parotid regions presents diagnostic challenges and adds complexity to the management. Herein, we present a case of a 23-year-old male diagnosed with NPC, initially presented with orbital metastasis followed by parotid gland involvement. The patient underwent surgical intervention along with chemoradiotherapy, resulting in complete remission without recurrence over a 12-month follow-up period.
