Trabeculectomy is still the gold standard anti-glaucoma surgery to control intraocular pressure. Its success, however, is impaired by undesirable subconjunctival fibrosis. Antimetabolites have been shown to enhance the success of filtration surgery by targeting the fibroblasts with their cytotoxic effect. But their anti-scarring effect comes at the cost of damage to the collateral structures, if not handled with care and caution. We describe a rare case series of three patients with endothelial decompensation following an uneventful bleb needling with mitomycin C.
