Destruens lung is a rare complication of various nosological entities, with Mycobacterium tuberculosis infection being responsible for 80% of the cases, and it occurs in patients who receive a delayed diagnosis and treatment. The imaging findings observed are a response to compensatory changes in the lung parenchyma, even though adequate management results in various complications following the destruction of the affected lung, which can be fatal in some cases due to the presence of hemoptysis.Likewise, the patient's quality of life is compromised due to being susceptible to opportunistic infections. We present a case of destruens lung secondary to M. tuberculosis infection, which is diagnosed in the context of a major hemoptysis episode.
