The human microbiota plays a crucial role in maintaining physiological homeostasis, influencing digestion, metabolism, immune function, and overall health. Probiotics, defined as live microorganisms that confer health benefits when administered in adequate amounts, have gained significant attention due to their ability to modulate the gut microbiome and host responses. This review provides a comprehensive overview of the role of probiotics in human health, focusing on their mechanisms of action, therapeutic applications, and emerging advancements. Probiotics exert beneficial effects through multiple mechanisms, including competitive exclusion of pathogens, production of antimicrobial compounds, enhancement of intestinal barrier integrity, modulation of immune responses, and generation of short-chain fatty acids. These mechanisms contribute to improved gastrointestinal health, particularly in conditions such as diarrhea, irritable bowel syndrome, inflammatory bowel disease, and lactose intolerance. Furthermore, probiotics play a vital role in immune system regulation by influencing cytokine production and enhancing antibody responses. Beyond gastrointestinal benefits, probiotics have demonstrated potential in managing metabolic disorders, improving oral and skin health, and regulating the gut–brain axis, thereby influencing neurological conditions such as anxiety and depression. Their role in disease prevention, including infections, cardiovascular disorders, and certain cancers, highlights their therapeutic relevance. Recent advances, including synbiotics, postbiotics, engineered probiotics, and novel delivery systems, have further enhanced their clinical applicability. However, challenges such as strain-specific variability, limited large-scale clinical validation, and regulatory concerns remain. Future research focusing on personalized probiotics and precision medicine approaches is essential to fully realize their therapeutic potential and ensure safe and effective integration into healthcare systems.
