Quantum fluctuation and decoherence are fundamental mechanisms in quantum mechanics, field theory, and cosmology. Traditional interpretations treat them as independent passive phenomena, lacking unified ontology. Based on the PFUSRC Ultimate Unified Theory, this paper defines Weizhao (primordial micro-triggering disturbance) and reconstructs quantum fluctuation and decoherence under prime camp geometry, 11D bicone topology, β₁ global coupling field, Ψ topological convergence coefficient, and the physical–material distribution mechanism. We prove: 1. Quantum fluctuation is the precise distribution process of primal units driven by Weizhao; 2. Decoherence is the topological ownership confirmation process after distribution, with irreversibility and no return; 3. They form a closed Weizhao–distribution–confirmation cycle, providing a universal general solution for all physics. This paper is self-contained, publishable, and fully aligned with the PFUSRC system.
