ABSTRACT Introduction: Thyroid hormones play an important role in the process of fat metabolism, so hypothyroidism may affect the development of fatty liver disease in nonalcoholic fatty liver disease (NAFLD). In some groups, there is a link between thyroid function and the prevalence of NAFLD. Objective: This study looked at the link between NAFLD and thyroid function. The study aims to determine the frequency of NAFLD in hypothyroid patients at King Abdulaziz University Hospital in Saudi Arabia. Materials and Methods: Data from 37 patients with hypothyroidism who visited endocrinology clinics between March 2021 and October 2023 were gathered. Analysis for NAFLD liver fat and stiffness measurements utilizing FibroScan® elastography equipment took place. Results: According to the collected data, 51% of participants had steatosis: 10.8% – Stage 1, 10.8% – Stage 2, and 29.7% – Stage 3. Patients with NAFLD showed higher blood pressure (BP), body mass index (BMI), aspartate aminotransferase, alanine aminotransferase, gamma-glutamyl transferase, and low platelet count. Participants with NAFLD had low free T4 levels. Conclusion: Patients with hypothyroidism have a higher risk of NAFLD with higher BMI, BP, lipids, and liver enzymes. Moreover, free T4 levels were found to be lower in NAFLD patients.
