In early May 2026, a severe outbreak of hantavirus, specifically Andes Virus, occurred aboard the Dutch cruise ship Motor Vessel Hondius, resulting in a total of 13 cases (11 confirmed and 2 probable) and 3 fatalities as of May 27. The infection likely originated from pre-embarkation rodent exposure in Patagonia and subsequently human-to-human transmission occurred onboard the ship based on the epidemiological investigation and viral sequence analysis. The Andes virus was confirmed as the causative agent. The epidemic risks of cruise ship environments, including recirculated air conditioning systems and confined spaces, may promote aerosol transmission and viral persistence. The hantavirus outbreak exposed severe weaknesses: ambiguous legal jurisdiction between flag and port states, onboard emergency response deficiencies, limited health surveillance and risk control. This rare and lethal maritime outbreak highlights critical gaps in maritime public health preparedness. It underscores the urgent need for enhanced critical research on related pathogens in maritime environments, upgraded maritime medical facilities and emergency response capacity, and improved public health education to prevent and mitigate similar infectious disease emergencies on cruise ships.
