Abstract

Introduction and Objective: This post hoc analysis evaluated the effect of semaglutide (sema) for weight management on blood pressure (BP) in adults with hypertension and overweight/obesity. Methods: Data were included from participants of STEP 1, 3, 5-9 (weekly injectable sema 2.4 mg vs placebo) and OASIS 4 (daily oral sema 25 mg) with overweight/obesity, without type 2 diabetes, and with uncontrolled hypertension at screening and baseline (systolic BP SBP ≥140 mmHg and/or diastolic BP DBP ≥90 mmHg). Changes in SBP and DBP were evaluated. Results: At baseline, hypertension criteria were met by 555/3755 (14.8%) of STEP and 42/202 (20.8%) of OASIS 4 participants, of which 46.7% (STEP sema), 47.1% (STEP placebo) and 54.8% (OASIS 4) were taking antihypertensive medication. Mean changes in SBP and DBP from baseline are shown in the Figure. From baseline to week 68, significant changes in SBP (estimated treatment difference ETD -5.48 mmHg, 95% CI -7.78, -3.19; p0.0001) and DBP (ETD -2.73 mmHg, 95% CI -4.19, -1.27; p=0.0003) were seen with sema compared with placebo in the pooled STEP trial data. Conclusion: Patients with uncontrolled hypertension and overweight/obesity experienced meaningful improvements in SBP following treatment with sema (injectable or oral) for weight management.
