Introduction and Objective: Weight loss interventions that combine pharmacotherapy with lifestyle change are foundational for the prevention and management of type 2 diabetes (T2DM) in people with overweight or obesity. However, evidence from large-scale, real-world integrated programs is limited. This study evaluated weight loss outcomes of a telehealth medical weight management program in patients with prediabetes (preDM) or T2DM. Methods: A retrospective observational study of patients with preDM or T2DM enrolled in a telehealth medical weight management program who had ≥1 year of subscription, initiated anti-obesity medication (AOM), requested ≥1 AOM refill, and had baseline and week 52 weights. Mean weight change overall and glucagon-like peptide-1 receptor agonist (GLP-1RA) subgroup and proportions reaching weight loss thresholds were computed at 26 and 52 weeks. Results: Patients with preDM (n=5,114; 88% female; mean BMI 38 kg/m2) and T2DM (n=736; 79% female; mean BMI 40 kg/m2) were included. Those with preDM lost 12.4% body weight (95% CI: -12.6%, -12.3%) at 26 weeks, and 18.0% (95% CI: -18.2%, -17.8%) at 52 weeks. Those with T2DM lost 12.8% body weight (95% CI: -13.3%, -12.4%) at 26 weeks, and 18.2% (95% CI: -18.8%, -17.6%) at 52 weeks. In a GLP-1RA subgroup, those with preDM (n=4,457) lost 18.7% (95% CI: -18.9%, -18.5%), and those with T2DM (n=669) lost 18.3% (95% CI: -18.9%, -17.6%) at 52 weeks. A high proportion lost ≥5% (95.8% preDM; 94.4% T2DM); ≥10% (84.7% preDM; 82.1% T2DM); ≥15% (64.0% preDM; 63.0% T2DM); and ≥20% body weight (39.5% preDM; 41.6% T2DM) at 52 weeks. Conclusion: In a real-world cohort of patients with preDM or T2DM, participation in a telehealth medical weight management program integrating AOM and behavioral support resulted in substantial weight loss at 1 year. Findings support integrated, remotely delivered models of care for meaningful weight loss in high metabolic risk populations.
