Abstract Piloleiomyomas are benign tumors of the arrector pili muscle that typically present as painful papules or nodules. Segmental distribution and interlesional morphological variation are uncommon and may represent cutaneous mosaicism. We report a case of multiple cutaneous piloleiomyomatosis in a 31-year-old man who presented with multiple painful, firm papules and nodules confined to the left side of the body, along with a solitary, distinct lesion on the face. The segmental distribution and variation in morphology suggest a mosaic pattern, specifically Type 5 (lateralization). Recognition of such atypical presentations may aid in early diagnosis and screening for associated syndromes such as hereditary leiomyomatosis and renal cell carcinoma.
