Abstract

Introduction and Objective: Teplizumab (TPZ) is the first disease-modifying therapy approved to delay progression from stage 2 (s2) to 3 T1D. Yet, little is known about TPZ use in real world (RW) practice. Methods: This mixed methods survey invited adult and pediatric HCPs with ≥1 US TPZ patients to complete a 1-hour, semi-structured interview that included elements of validated scales (Oct-Nov 2025). Interviews queried HCPs on TPZ initiation and treatment, care coordination and operational logistics, and adverse event management. Scales scores were calculated and interviews analyzed with this preliminary thematic analysis. Results: Qualitative findings from 39 HCPs identified 5 main themes: 1) HCPs want to offer TPZ to give patients more treatment options, 2) TPZ includes a time-consuming insurance approval process with varying lab requirements, 3) current s2 eligibility limits access; there is a need for earlier and expanded T1D screening, 4) infusion location and capability was logistically difficult, yet HCPs consistently emphasized TPZ’s evidence of beneficial, clinically meaningful outcomes, 5) HCPs thought the clinical value of TPZ outweighed challenges and expressed optimism about expanding future use. Table 1 contains survey results. Conclusion: This study provides insights into RW TPZ infusion experiences, highlighting enthusiasm for TPZ, implementation challenges, adoption facilitators and barriers, and promising implementation practices.
