Introduction and Objective: Early stage T1D screening programs in routine clinical care are in their infancy. With the availability of teplizumab, a disease-modifying therapy that delays progression to Stage 3 T1D, there is an urgent need to reimagine T1D care. Understanding current screening practices can inform implementation strategies and identify gaps as the field moves toward early detection and intervention. Methods: A cross-sectional survey was administered to 64 centers participating in the T1DX-QI Collaborative; the survey was open from August to October 2025. Survey responses were analyzed using descriptive statistics to characterize current screening practices, autoantibody testing approaches, and readiness for teplizumab administration. Results: Responses were received from 63 centers (98% response rate). Among respondents, 67% were pediatric centers and 33% were adult centers. Survey results are displayed in Table 1. Conclusion: T1D screening programs are emerging but remain more common in pediatric than adult centers. Although many sites report initiation of screening activities, autoantibody testing is frequently conducted through research-based platforms with variable integration into routine clinical workflows, highlighting the opportunity for standardized approaches to early detection and intervention in clinical care.
