Abstract Recurrent respiratory papillomatosis presents significant challenges in airway management, particularly when lesions are pedunculated. Here, we report a case of adult-onset recurrent respiratory laryngeal papillomatosis presented with breathing difficulty. A pedunculated mass was obstructing the glottis during inspiration and dislodged during expiration. Awake tracheal intubation using fibreoptic bronchoscopy remains the preferred approach, allowing controlled navigation around obstructive mass while preserving patient ventilation and cooperation. The different modes of presentation, options for airway management and other concerns are discussed.
