The global dietary supplements market was valued at approximately USD 177 billion in 2023 and is projected to exceed USD 300 billion by 2028. Billions of people around the world take vitamin, mineral, herbal, or protein supplements daily — many without evidence that they need them, and some in ways that may be actively harmful. Simultaneously, genuine nutritional deficiencies affect large populations worldwide: vitamin D deficiency affects over one billion people globally; iodine deficiency remains the leading preventable cause of intellectual disability; B12 deficiency is endemic among vegetarian and vegan populations; and iron deficiency anaemia affects an estimated 1.62 billion people. The question of who needs supplements, which ones, at what doses, and at which life stage is therefore not a simple one. This article provides a systematic, evidence-based review of supplementation across four life stages: childhood and adolescence (5–18 years), young adulthood (19–35 years), middle age (36–55 years), and later life (56 years and above). Drawing on guidelines from the World Health Organization, the National Institutes of Health Office of Dietary Supplements, the Indian Council of Medical Research (ICMR), and Ayurvedic Rasayana traditions for age-appropriate nutritional support, the article evaluates the evidence for and against the most commonly consumed supplements — Vitamin D, Vitamin B12, Omega-3 fatty acids, Iron, Magnesium, Zinc, Calcium, and Protein — across each life stage. The conclusion argues that most healthy adults eating a varied whole-food diet do not need most supplements, that testing before supplementing is the appropriate protocol, and that targeted supplementation for documented deficiency is the evidence-based standard.
