This conceptual note argues that immune-cell presence is not equivalent to immune performance. Immune cells must be interpreted not only by their identity and abundance, but also through their functional state, tissue context, antigenic history, exhaustion, memory, reversibility and capacity for functional rescue. The paper develops a dynamic framework moving from static cell identity toward functional immune state, dynamic transition, recovery or rescue potential and pathogenic memory. It examines T-cell exhaustion as an example of immune presence without effective performance, rescue as a state-dependent possibility rather than simple immune activation, and pathogenic memory as an immune architecture capable of preserving recurrence risk across periods of apparent clinical improvement. The analysis extends four principles developed in the URM / Dynamic Medicine series: response is not the state, diagnosis is not the trajectory, remission is not necessarily reset, and presence is not performance. It considers their implications for chronic inflammation, autoimmune disease, cancer immunology, infection, immunotherapy, relapse, remission and precision medicine. The central argument is that immune-cell identity is necessary but insufficient. Future immune interpretation must also assess what the immune system is capable of doing at a given time, whether dysfunctional states remain reversible, what pathogenic memory persists, and toward which state the system is moving.
