A bstract Frontal lobe seizures may present with complex hypermotor behaviors and autonomic features, often leading to diagnostic confusion. Here, we report a case of a 10-year-old boy presenting with recurrent stereotyped episodes of sudden running, vocalization, and urge to void, initially misdiagnosed as a urological disorder. Furthermore, clinical evaluation revealed stereotyped semiology with impaired awareness and brief postictal sluggishness. Video-electroencephalogram (EEG) captured typical events demonstrating ictal onset from mid-frontal regions with bilateral propagation. Initiation of oxcarbazepine resulted in complete seizure control. This case report highlights the importance of considering epilepsy in children presenting with paroxysmal behavioral or autonomic events, particularly when episodes are stereotyped, frequent, and sleep-related. Early recognition and video-EEG evaluation are essential to avoid diagnostic delay and ensure appropriate treatment.
